This form allows you to submit a request to Therapy Box for a FREE ElevenLabs Voice Clone. In order to qualify, you must have non-temporary speech loss caused by a diagnosed disease and be above the age of 18.

This form must be completed by a professional on your behalf.

Once you have submitted this form, the Therapy Box team will review your submission to make sure you qualify and then will be able to allow access through the ElevenLabs Impact Program. Please ensure that you have created a FREE ElevenLabs account before completing this form, as you will need to provide the account email below.
